Overview

What is Orchiectomy Surgery?

Orchiectomy is the surgical removal of the testes. For patients pursuing gender-affirming care or decreased testosterone levels, bilateral orchiectomy eliminates over 95% of endogenous testosterone production, which can simplify or reduce hormone therapy requirements. The procedure may also be pursued for personal comfort, chronic pain, dysphoria reduction, or as a step toward further gender-affirming surgeries.

Procedure options include:

  • Orchiectomy without scrotal excision — testicular removal with scrotal tissue preserved
  • Orchiectomy with scrotal excision — removal of the testes and scrotal skin for a smoother contour
  • Orchiectomy with testicular implants — placement of prosthetic implants for patients who wish to retain external appearance

A comprehensive consultation allows Dr. Sherie to evaluate your anatomy, discuss your goals, and determine the most appropriate approach for your individual plan.

Who seeks orchiectomy

Why Choose Orchiectomy

Orchiectomy can be an important choice for people who want to reduce testosterone, lessen the need for long-term hormone blockers, relieve discomfort or dysphoria, or have a body that feels more aligned with who they are. It can also support those seeking a more neutral genital appearance, or anyone looking to simplify their hormonal care and overall well-being.

There is no single “right” reason — only what feels right for you.

Transfeminine (MTF / AMAB)

Transfeminine Patients

An affirming step that lowers testosterone at its source — often reducing the need for anti-androgens and supporting feminizing hormone therapy.

Cisgender Men

Orchiectomy for Cisgender Men

Some cisgender men pursue orchiectomy to address chronic pain, manage hormonal imbalance, or find relief from concerns related to their testicles or testosterone production.

Non-Binary

Non-Binary Patients

Non-binary individuals may seek orchiectomy for a range of reasons — the desire for a more neutral anatomy, relief from dysphoria, or to simplify hormonal care.

Surgical options

Orchiectomy Surgical Options

During consultation, Dr. Sherie will talk through each option with you — always validating, and ensuring the outcome aligns with your personal vision of yourself.

Scrotal-preserving

Orchiectomy Without Scrotal Excision

This approach removes the testes while preserving the scrotal tissue. Scrotal preservation may be appropriate for patients who prefer to maintain external appearance, who are considering future vaginoplasty, or who prefer a less extensive initial procedure.

The scrotal skin remains but will typically tighten and retract over time. All skin is preserved and can be used for future vaginoplasty reconstruction.

Smooth contour

Orchiectomy With Scrotal Excision

For patients who prefer a smoother, more minimal genital contour and do not plan to use scrotal tissue for future reconstruction, this approach removes both the testes and the scrotal skin — leaving a clean, smooth surface along the perineum.

Incision placement is carefully planned. Over time, the scar is nearly invisible — appearing as a single fine line in the midline perineal space.

Retained appearance

Orchiectomy With Testicular Implants

For patients who wish to retain a typical scrotal appearance following testicular removal — whether for personal, professional, or social reasons — prosthetic testicular implants can be placed at the time of surgery.

Implant size and placement are individualized to achieve natural-looking symmetry and proportion.

How Technique Selection Is Determined

Technique selection is evaluated based on several factors during consultation. There is no universal approach — planning is fully individualized.

Goals for scrotal tissue — preservation, excision, or implant placement

Plans for future gender-affirming procedures, including vaginoplasty

Personal preference for external genital appearance

Overall surgical staging and recovery planning

Hormone therapy history and anticipated future needs

Combined procedures

Staging With Top Surgery or Other Procedures

For patients pursuing multiple gender-affirming procedures, orchiectomy may be planned as a standalone procedure or staged alongside other surgeries. Combined surgical planning is discussed in detail during consultation.

Patients considering top surgery (chest or breast) along with orchiectomy may benefit from coordinated staging — either performing procedures simultaneously where appropriate, or sequencing them to optimize recovery and results. Dr. Sherie provides individualized recommendations based on your surgical goals and overall health.

Recovery

Recovery After Orchiectomy

Orchiectomy is typically performed as an outpatient procedure under general anesthesia at Dr. Sherie’s AAAHC-accredited facility in Charlotte, NC. Recovery is generally straightforward, with most patients returning to daily activities within a short time.

Swelling and bruising resolve. Most patients return to light daily activities.

Restricted lifting

Restricted strenuous activity and lifting. Ongoing care with Dr. Sherie's team.

Full activity return

Gradual return to full activity. Ongoing guidance and scheduled follow-up appointments throughout recovery.

Hormone therapy

Effects on Testosterone & Hormone Therapy

Bilateral orchiectomy eliminates over 95% of endogenous testosterone production, which often allows for simplification of feminizing hormone therapy. The half life of testosterone in the blood is less than one hour; and within 24 hours of orchiectomy, the patient’s testosterone levels are within “castrate level” and testosterone blocking medications may be discontinued.

Any adjustments to hormone therapy should be coordinated with your prescribing provider. Dr. Sherie’s team can provide surgical documentation to support this coordination — and for patients who prefer to manage their hormone care in one place, The Cosmetic Concierge also offers  bio-identical hormone pellet therapy on-site.

BioTe Hormone Therapy — Available On-Site

BioTe uses pellet-based bio-identical estrogen and/or testosterone hormone delivery to provide consistent, sustained hormone levels, without the fluctuations that can come with other delivery methods. For patients who have undergone orchiectomy, this means hormone management handled by a team that already knows your surgical history, your goals, and your care plan.

Patients interested in combining surgical care with ongoing hormone management through BioTe are encouraged to ask about this option during their consultation.

Frequently Asked Questions

Will orchiectomy affect my hormone therapy?

Bilateral orchiectomy eliminates over 95% of endogenous testosterone production, which often allows for simplification of feminizing hormone therapy. The half-life of testosterone in the blood is less than one hour — within 24 hours of orchiectomy, testosterone levels reach castrate level and testosterone-blocking medications may be discontinued. Yes. Scrotal-preserving orchiectomy is available for patients who prefer to maintain scrotal tissue — whether for future surgical use, personal preference, or other reasons. Scrotal skin will typically retract and become less prominent over time, but all the skin remains and can be used for future vaginoplasty.

Depending on the vaginoplasty technique, scrotal tissue may be used in reconstruction. If vaginoplasty is part of your longer-term surgical plan, this should be discussed during consultation so that staging and tissue preservation can be planned accordingly.

Incision placement is planned to minimize visible scarring. For scrotal-preserving procedures, a single small incision is made in the midline (raphe) of the scrotal skin. For scrotal excision, a single fine line is left in the midline perineal space — over time this is nearly invisible.

For some patients, staging orchiectomy alongside top surgery or other gender-affirming procedures may be appropriate. This is evaluated individually based on your surgical goals, recovery capacity, and overall health. Combined planning is discussed in detail during consultation.
