Overview
What is Top Surgery?
Elective Mastectomies (aka Top Surgery) are surgical breast removal done by patient choice. It is often called “Top surgery” when the cosmetic goal is to achieve an androgynous (non-binary) or masculine appearance. At The Cosmetic Concierge in Charlotte, NC, Dr. Hope Sherie approaches every surgical plan with careful anatomical planning, attention to proportion, and a commitment to natural-looking results.
Dr. Sherie offers several top surgery techniques which are discussed and selected during your consultation based on your aesthetic goals and chest anatomy. Often patients are candidates for more than one surgical technique.
Procedures may include:
Removal or reduction of breast tissue
Skin tightening and chest wall reshaping
Nipple and areola resizing, repositioning, or preservation
Breast volume enhancement through implants
VASER liposuction or fat transfer for contour refinement
A comprehensive consultation allows Dr. Sherie to evaluate your anatomy and determine the safest, most effective approach for your goals.

Top Surgery Techniques
Dr. Sherie offers several top surgery techniques which are discussed and selected during your consultation based on your aesthetic goals and chest anatomy.
Nipple-preserving
Double Incision — Buttonhole Technique
The buttonhole technique preserves the natural nipple-areolar complex by maintaining a pedicle attachment while still allowing for full tissue removal. This approach may be appropriate for patients who desire sensation preservation and meet the anatomical criteria for this method.

Most common double incision
Double Incision — Nipple Graft
For patients with more significant tissue volume or excess skin, a double incision with free nipple graft allows for complete contouring and full skin adjustment. The nipple-areolar complex is resized and repositioned as a graft, providing precise aesthetic control over final placement and diameter.

Clean contour
Double Incision — No Nipples
For patients who prefer to omit nipple reconstruction, double incision without nipple preservation offers a clean chest contour with no additional nipple-related procedures. This approach is discussed and planned according to personal preference during consultation.

Limited incision
Keyhole (Peri-Areolar) Technique
In select cases with smaller tissue volume and strong skin elasticity, a keyhole technique may be appropriate. This limited-incision approach allows for tissue removal while preserving the natural nipple attachment.

How Technique Selection Is Determined
Technique selection is evaluated based on several factors during consultation. There is no universal approach — planning is fully individualized.
Amount of breast tissue present
Degree of skin elasticity and expected skin retraction
Nipple position, size, and desired final placement
Preferred scar location and orientation
Overall chest width and body proportions
Patients with moderate to significant tissue volume or excess skin are typically best served by a double incision approach. Patients with smaller amounts of tissue and strong skin quality may be candidates for limited-incision techniques.
Add-on procedures
Lateral Chest & Upper Body Contouring Add-Ons
For patients seeking more comprehensive reshaping, Dr. Sherie offers optional contouring additions to double incision procedures:
½ Lateral Chest Lift
Targeted liposuction of the side chest to match the thickness of the front, with minor extension of the incision as needed to avoid “dog ears”.
Full Lateral Chest Lift
Full excision of excess skin and fat (sometimes called “the bra roll”) on the side chest to avoid “dog ears” for those with more side tissue.
Upper Body Lift
Excision of excess skin and fat along the side of the chest, extending to the upper back, which can remove a full “roll” of tissue that continues onto the back. This technique includes turning the patient onto the stomach (while under anesthesia) and performing liposuction of the back in addition to excision of any excess skin that extends to the sides of the upper back.
Revision surgery
Top Surgery Revision
If you have had previous chest surgery and are seeking refinement, revision procedures can address contour irregularities (areas which cave in or stick out as compared to the surrounding tissue), scar concerns, asymmetry, residual tissue, nipple or areolar revisions or reconstruction after nipple loss. Revision planning is fully individualized and focused on improving overall balance and smoothness.
Advanced contouring
Additional Contouring Options
In some cases, top surgery may be complemented by additional procedures to refine surrounding areas and enhance overall upper body proportions:
Liposuction — for chest, underarm, or upper back refinement (this is included with lateral chest and upper body lift procedures).
Fat transfer — for contour enhancement or volume balancing
Pectoral implants for pre-existing muscle deformities
These options are discussed thoughtfully during consultation to determine whether they would meaningfully improve your result.

Questions & answers
Frequently Asked Questions
Am I a candidate for keyhole top surgery?
Keyhole techniques are typically appropriate for patients with minimal tissue volume and strong skin elasticity. Candidacy is evaluated during consultation based on a thorough anatomical assessment. Patients who do not meet keyhole criteria may be better served by a double incision approach.
What is the buttonhole technique, and how is it different from a nipple graft?
The buttonhole technique preserves the nipple-areolar complex by maintaining a tissue attachment (pedicle), which may allow for some degree of sensation preservation. A free nipple graft removes and repositions the nipple as a graft, offering more precise control over final placement. The appropriate technique depends on anatomy and is discussed during consultation.
How visible are scars after top surgery?
Scar placement depends on the technique used. Double incision scars are typically positioned along the lower chest and mature significantly over 12–18 months. Incision planning is designed to create clean, well-positioned scars, and Dr. Sherie provides guidance on scar care throughout recovery.
Can top surgery be combined with other procedures?
In select cases, liposuction, fat transfer, or lateral chest contouring may be performed at the same time to enhance overall upper body proportion. This is evaluated individually and recommended only when it would meaningfully improve the result.
How long until I see final results?
Initial contour is visible once swelling begins to resolve, typically within the first few months. Final contour refinement and scar maturation develop over several months. Most patients see a close-to-final result by 6–12 months post-operatively.
